New Progress -- XellSmart Unveiled Phase I Results of Off-the-Shelf Parkinson's Disease Cell Therapy at MDS & Completed Blinded Phase II Enrollment

SEOUL, South Korea and SHANGHAI, Oct. 18, 2026 /PRNewswire/ -- New clinical data for XellSmart's off-the-shelf cell therapy for Parkinson's Disease was presented at the International Congress of Parkinson's Disease and Movement Disorders (MDS), one of the world's most prestigious gatherings for Parkinson's and movement disorder research. During the session Parkinson's Disease: Clinical Trial, Pharmacology, and Medical Management, an investigator shared landmark Phase I results for XS411, XellSmart's proprietary off-the-shelf cell injection, alongside a major milestone: full enrollment in its multicenter, randomized, parallel-group Phase II trial in China, which uses an open-label design with blinded endpoint assessment. The findings captured widespread interest among the global movement disorders community.

XS411 is an allogeneic cell therapy candidate comprising induced pluripotent stem cell (iPSC)-derived dopaminergic neural progenitor cells. It is designed for use without patient-specific human leukocyte antigen (HLA) matching and is administered with a standardized immunosuppressive regimen. Following transplantation into the putamen, the cells are intended to differentiate into functional dopaminergic neurons. These cells restore dopamine synthesis and metabolism, delivering meaningful improvements in Parkinsonian symptoms while maintaining a favorable safety profile.

Phase I Findings: Sustained Improvements in Motor Function, QoL and Molecular Imaging Biomarkers

This Phase I single‑arm, open‑label trial enrolled nine participants with moderate‑to‑advanced Parkinson's disease into three dose cohorts using a 3+3 dose‑escalation design. Mean disease duration was approximately 10 years. All had end‑of‑dose wearing‑off and OFF‑state Hoehn–Yahr stages of 3 to 4. Symptoms remained inadequately controlled with standard‑of‑care treatment, causing marked impairment in occupational and daily function.

Following standardized screening and assessments, participants received XS411 stem cell transplantation into the bilateral posterior putamen. The trial met its primary safety and tolerability objectives, and XS411 was generally well tolerated. No dose-limiting toxicities, serious adverse events or adverse events considered related to XS411 were reported during the follow-up period. Serial MRI monitoring showed no aberrant cellular overgrowth or tumor formation, and no graft-induced dyskinesia was observed.

All nine participants achieved robust clinical improvements during the follow-up period, with response magnitudes surpassing published minimal clinically important difference (MCID) thresholds — showing that the motor benefits potentially carry clinical significance.

For motor function, participants' OFF-state MDS-UPDRS Part III scores improved by an average of more than 30 points from baseline at six months post-transplant. Therapeutic gains continued to deepen per available longer-term follow-up data, with a trend toward greater efficacy in the higher-dose group compared with the lower-dose cohort.

Patient diaries captured real-world motor fluctuations,a major burden for people with moderate-to-advanced Parkinson's disease. At six months post-transplant, participants gained an average of over five hours of good ON time per day— defined as ON time without troublesome dyskinesia. Treatment benefits were sustained throughout the available follow-up, with a trend favoring the higher-dose arm.

Patient-reported outcomes from the 39-item Parkinson's Disease Questionnaire (PDQ-39) mirrored these positive trends. The results corroborate clinician-rated motor function and patient diaries, confirming concordance between measurable motor improvements and participants' self-perceived gains in daily living.

Notably, however, open-label trials carry inherent risk of bias in clinician ratings and patient-reported outcomes. To mitigate this limitation, this Phase I study integrated ¹⁸F-DOPA PET molecular imaging — a globally accepted objective biomarker to assess dopamine synthesis and metabolic capacity.

Post-transplant ¹⁸F-DOPA PET scans revealed elevated specific binding ratios in the putamen compared with baseline. Areas of increased tracer signal were anatomically consistent with the injection sites. These data demonstrate that transplanted XS411 cells survive, engraft and differentiate at the target site to establish functional dopamine synthesis and metabolic capacity. The graft replaces degenerated dopaminergic neurons with healthy, functionally active cells, creating a new endogenous source of dopamine.

Phase II Full Enrollment Achieved for World's First PROBE-Designed Stem Cell Trial in Parkinson's Disease

Building on the promising Phase I clinical outcomes, the registrational Phase II study of XS411 has successfully completed enrollment. This multicenter trial investigates the efficacy and safety of off-the-shelf XS411 cell injection for the treatment of primary Parkinson's disease. Led by Beijing Tiantan Hospital, a flagship National Center for Neurological Disorders, the trial includes multiple top-tier Parkinson's treatment centers across China as participating sub-centers, namely Peking Union Medical College Hospital, the Second Affiliated Hospital of Soochow University, and Jiangsu Province Hospital.

To strengthen the robustness of evidence, the trial employs a Prospective Randomized Open-label Blinded Endpoint (PROBE) design. Its core methodological features—randomized grouping, parallel-arm comparison, and blinded endpoint evaluation—effectively mitigate research bias, enabling more rigorous assessment of therapeutic efficacy and providing solid evidence to support subsequent confirmatory clinical trials. Specifically, under the study's blinded endpoint framework, independent outcome assessments eliminate subjective investigator bias, delivering a more precise and reliable reflection of genuine disease progression and treatment effects.

Notably, this is the world's first stem cell therapy trial for Parkinson's disease built around the PROBE design. To date, all 30 planned participants have been enrolled, cell transplantation has been completed in the treatment group, and follow-up is ongoing. The data generated from this study is expected to lay solid clinical and methodological groundwork for the pivotal Phase III registrational trial and provide high-grade evidence to support clinical translation of this novel cell therapy.

Delving into XS411: an HLA-Independent Off-the-Shelf Cell Therapy for Parkinson's Disease

Recent landmark studies published in Nature (2025), Cell (2025), and Nature Medicine (2026) have demonstrated the transformative potential of iPSC-derived dopaminergic neural progenitor cells for Parkinson's disease (PD) treatment. Among the four pioneering international trials, three adopted embryonic stem cell-based approaches, which face inherent ethical limitations that restrict broad clinical application. Kyoto University's iPSC therapy, the only conditionally approved PD cell product to date, relies on HLA "super-donor" lines and encounters technical, cost-related and regulatory barriers hindering worldwide deployment.

Conversely, XellSmart's XS411 is an off-the-shelf cell injection eliminating patient-specific HLA matching. Engineered for broad accessibility, XS411 avoids personalized cell production and large HLA cell banks. Combined with standardized immunosuppression, it has exhibited preliminary safety and promising efficacy for PD cell replacement in the Phase I trial.

But this is not enough. Off-the-shelf neural cell therapies demand reproducible manufacturing and accumulating clinical evidence. This design, only when combined with standardized cell sources, GMP-grade manufacturing and robust QC, could enable stable multicenter supply for future widespread clinical use. XS411 offers preliminary validation of this technical toute: Phase I dose-escalation is complete, and a multicenter randomized Phase II trial is underway, supporting a feasible route toward large-scale translation.

Further, two key milestones in 2026 bolster XS411's standardized global development.

First, XS411 has secured funding under China's National Major Innovative Drug Program. The XellSmart-led consortium includes Tongji University, NIFDC, Beijing Tiantan Hospital and Peking Union Medical College Hospital. Kicked off in June 2026, this industry-academia-clinic collaboration will speed breakthroughs and translation of universal neural cell drugs.

Second, in July 2026, after clinical trial clearance in China and the US, XS411 obtained FDA Fast Track Designation. This milestone enables efficient FDA communication on clinical strategy and trial design, and may qualify the program for expedited pathways including rolling review.

Leveraging these milestones and robust China clinical progress, XS411 maintains global competitiveness as an innovative, widely accessible cell therapy candidate through parallel progress across three workstreams: clinical evidence generation, pharmaceutical quality system optimization, and cross-border registration capacity building.

From Bench to Bedside: Full‑stack R&D and Manufacturing Sustain Global Rollout

The competitiveness of cell therapeutics spans the entire value chain, from cell source to clinical administration. Built around XS411, XellSmart has established an integrated R&D system covering iPSC reprogramming, directed differentiation, product characterization, quality control and preclinical safety assessment to manufacture clinical-grade cell products. Analytical tools including single-cell transcriptomics and systematic safety evaluations jointly support product characterization and quality research.

This capability is being validated within multicenter clinical settings.

The World Travel & Tourism Council (WTTC) serves as collaboration organisation, while the Macao Government Tourism Office and the China Chamber of Tourism serve as co-organisers, with the Global Tourism Economy Research Centre acting as coordinator.

The event also receives strong support from key official and international entities, including the Ministry of Culture and Tourism of the People's Republic of China, the All-China Federation of Industry and Commerce (ACFIC), the Liaison Office of the Central People's Government in the Macao Special Administrative Region, the Office of the Commissioner of the Ministry of Foreign Affairs of the People's Republic of China in the Macao Special Administrative Region, the Pacific Asia Travel Association (PATA), the European Travel Commission (ETC), the World Tourism Alliance (WTA), the World Tourism Cities Federation (WTCF), the International Mountain Tourism Alliance (IMTA), the Sino-International Entrepreneurs Federation (SIEF), the China Women's Chamber of Commerce (CWCC), the China Tourism Association (CTA), the Commerce and Investment Promotion Institute, and the Association of Chinese Enterprises in Macao (ACEM). Together, these organizations unite global collaborative strength to champion the sustainable development of global cultural tourism.

In his welcome remarks, HO Hau Wah, Vice Chairman of the National Committee of the Chinese People's Political Consultative Conference, and Forum Chairman of Global Tourism Economy Forum, stated: "This year marks a major milestone in the evolution of the Forum. Last December, the 11th Forum ventured beyond Macao for the first time, hosting a highly successful event in Harbin, Heilongjiang, and achieving a historic breakthrough as the Forum expanded nationwide. Building on this momentum, this year's Forum centers on the timely theme 'Crisis, Resilience, Elevation: Reshaping a Sustainable Global Future through Tourism Economy'. As an international platform deeply rooted in Macao, the Forum actively aligns with the Special Administrative Region's overarching development, continually injecting new dynamic momentum into Macao's economic diversification."

SAM Hou Fai, Chief Executive of the Macao Special Administrative Region, delivered welcome remarks, stating: "Since its inception in 2012, the Global Tourism Economy Forum has become an important platform for high-level dialogue and multilateral cooperation in global tourism. Macao will actively leverage its unique strengths of 'backing by the Motherland and connecting to the world', while advancing the 'Northward, Southward, Westward and Eastward' strategies. Furthermore, the SAR will spare no effort in promoting the development of the World Centre of Tourism and Leisure, alongside the exchange and cooperation base with Chinese culture as the mainstream and co-existence of diverse cultures. We look forward to working with all partners through this year's Forum to extend the momentum of the tourism economy into wider domains, while fostering a collaborative landscape for mutual benefit and driving sustainable innovation across the global industry."

Shaikha AL NUWAIS, Secretary-General of UN Tourism, delivered opening remarks, stating: "So far this year, 690 million people have travelled internationally – 3 million more than during the same period last year. With the geopolitical situation and with everything happening in the Middle East, this number is very promising, and this tells us that the tourism sector has tremendous potential. But success is not only about how many people travel. It's about what their journeys create for destinations and for communities. As tourism evolves and travellers' expectations rise, we must equip our workforce with the skills, professionalism, and service mindset to deliver the quality today's travellers' demand. Macao has an important story to tell. Macao is moving from volume to value, connecting culture, gastronomy, health and innovation, as a World Centre of Tourism and Leisure."

LONG Yongtu, Former Vice Minister of Ministry of Foreign Trade & Economic Cooperation of China, and Former Secretary-General of Boao Forum for Asia, delivered a keynote speech centered on "China's Model of Inclusive Growth", stating: "The deep integration of tourism with other sectors expands the foundation of tourism development while strengthening industry resilience. Inclusive tourism directly benefits communities, earning the sector broad and societal support. At the same time, advancing environmentally friendly tourism enhances the destination's appeal and elevates a nation's global reputation. Through collective strength, tourism can become an open, inclusive, and sustainable force that meaningfully contributes to global economic growth, social stability, and human well-being."

Gloria GUEVARA, President & Chief Executive Officer of World Travel & Tourism Council (WTTC), and Honorary Chairman of GTEF, delivered opening remarks, stating: "We do a lot of research, showing that China is driving global tourism growth. One key policy is the visa exempt, which make it easier for more travelers to come. Getting to China is easier through improved connectivity, with flights and transportation connecting China to the world. Although we have a lot of situations around the world, and people are worried, I believe that through public and private collaboration and working together, we can unlock future growth and build a better future."

LUO Laijun, Vice Chairman of All-China Federation of Industry and Commerce, delivered congratulatory remarks, stating: "Recent years have seen global tourism weather severe headwinds from public health emergencies and extreme weather to economic volatility and shifting consumer trends, all of which have accelerated the industry's structural transformation. By focusing on emerging consumer trends and driving breakthroughs in cultural integration, green transition, and smart travel, the industry can turn crises into catalysts for evolution."

Representing the Partner Country, Miguel SANZ CASTEDO, Representative of Minister for Industry and Tourism of Spain, Director General of Turespaña, and President of the European Travel Commission, delivered partner country remarks, stating: "Spain and China are among the world's leading tourism powers. We face common challenges: how to create greater value through tourism? How to harness technology while preserving the human dimension of travel? And how to ensure that tourism contributes to prosperity and wellbeing? These challenges require dialogue and cooperation, which is why this Global Tourism Economy Forum is so valuable. Spain stands ready to work with our partners, in Macao, in the rest of China and beyond, to build a tourism sector that is more sustainable, more innovative, more connected and more human."

Representing the Featured Chinese Province, Bateer, Executive Vice Governor of the Hainan Provincial People's Government, delivered featured province remarks, stating: "As the World Centre of Tourism and Leisure, Macao stands as a glowing showcase of centuries-old Sino-Western cultural fusion, offering rich lessons from which Hainan can learn. As China's largest special economic zone and sole free trade port, Hainan shares the South China Sea with Macao, bound by geographic proximity, cultural affinity, and deep bonds of kinship that create vast potential for cultural tourism collaboration. As the Forum opens, global partners are warmly invited to Hainan for leisure and investment, embrace the new opportunities in the Hainan Free Trade Port, and jointly shape a more open, inclusive, and dynamic global tourism economy."

Re-emerging from its founding city of Macao, the 12th Global Tourism Economy Forum not only builds a practical bridge for global tourism collaboration, but also initiates a vital path toward overcoming industry uncertainty and reshaping a sustainable future. At a pivotal moment marked by the rebuilding of sector resilience and the pursuit of high-quality growth, the Forum will continue to bring global stakeholders together to enhance industry governance. By fostering collaboration through openness and driving breakthroughs through innovation, the event aims to chart a new blueprint for global tourism defined by strength, adaptability, and long-term value.

Beware the Tax Man: Ken Jennings Wants to Tax Every Washingtonian
Image File

SEATTLE, Oct. 17, 2026 /PRNewswire/ -- Vote Yes Repeal the Income Tax today released a new video ad featuring Jeopardy star and 'Tax Man' Ken Jennings, who endorsed Washington's new income tax, which erased 79-year-old pension protections for public workers. It doesn't take a quiz show host to know that putting the hard-earned retirement savings of teachers, firefighters, and police officers in jeopardy is wrong for Washington.

The law also fails to shield Social Security benefits, leaving retirees' Social Security checks exposed. Olympia already swept roughly $4 billion from the Law Enforcement Officers' and Firefighters' Retirement System Plan 1 (LEOFF 1) pension fund to plug budget gaps elsewhere. These aren't wealthy people, so why would lawmakers remove these protections? Because this tax is coming for everyone.

Voting Yes on I-645 repeals the income tax, restores the protections Olympia erased, and puts the power back in voters' hands.

As AI Speeds Up Marketing, Louisville's RedTag Bets on Strategy

Independent agency unveils new brand and website, and co-hosts Louisville's PROOF AI Workshop

LOUISVILLE, Ky., Oct. 17, 2026 /PRNewswire/ -- RedTag, an independent Louisville-based marketing agency, this month unveiled new positioning, marking its evolution from a digital performance agency into a strategy-first marketing partner. The move comes as AI makes execution faster than ever, and RedTag is betting that strategy, not speed, is what will set businesses apart. Clients can now choose from outcome-based programs that best fit their marketing needs.

"Clients don't come to us needing more tactics. They come to us needing a path," said Russ Renbarger, founder and CEO of RedTag. "Our new brand says out loud what our work has shown for years: We start with strategy and build everything on that foundation."

RedTag built its reputation on high-performing paid digital campaigns. Over the years, the agency expanded into strategy, creative, video production and website development, giving clients one partner for the full scope of their marketing. Most recently, a team of automation specialists has integrated AI across the agency to speed up production and analysis, guided by an internal AI policy that keeps people accountable for quality. The time AI saves goes back into strategy: knowing what to say, who to say it to and why.

"Our best client relationships have always been partnerships, where we're in the room when the big decisions get made," said Alison Conway, chief client officer at RedTag. "This evolution is about bringing that strategic partnership to every client."

On Oct. 14, RedTag co-hosted the PROOF AI Workshop with Slingshot, Vsimple and Advanced Business Solutions. The hands-on event gave Louisville business leaders practical experience applying AI to their own work. RedTag led the event's AI in Marketing and Branding track, with four sessions ranging from AI search optimization to a playbook for adopting AI across an organization.

"AI can produce hundreds of ads in an afternoon, but it can't tell you which one your brand actually needs and why," Renbarger said. "The faster the tools get, the more valuable human-led strategy becomes."

RedTag was named the agency of record for the Commonwealth of Kentucky in 2025. Its client list includes PGA of America, Castle & Key Distillery, Bourbon Barrel Foods, ShopCaterpillar and American Beverage Marketers. Since 2023, RedTag's work has received two Emmy Awards and more than 35 ADDY and AIGA awards. The agency is a four-time Louisville Business First Fast 50 honoree and was named one of the 2026 Best Places to Work in Kentucky.

Dispelling the Myth That ‘Cyber Insurance Doesn't Pay Claims’

Dispelling the Myth That ‘Cyber Insurance Doesn't Pay Claims’

“Cyber insurance doesn’t pay claims.”

This is a common perception in our industry that stems from a range of factors, including misunderstandings about claims data, differences in coverage and policy terms, and incomplete information. By taking a closer look at the underlying sources and context, brokers can help clients develop a more informed view of how cyber insurance responds when an incident occurs.

It’s easy to dismiss this myth as ignorance. Cyber insurance pays out billions of dollars in claims every year, and policies provided by Coalition paid more than $315 million in claims in 2025 alone.

But the danger for our industry is that the notion slowly accumulates and develops into a sales objection rooted in misunderstanding among insurance buyers. That reluctance can make it harder for businesses to evaluate whether cyber insurance is right for them, especially when coverage decisions are made during a brief conversation with their broker.

When clients raise this objection, brokers can provide context to control the narrative and rebut the myth that cyber insurance doesn’t pay claims. Below, we’ll explore how to overcome misinterpreted data and fabricated statistics to explain how cyber insurance is designed to respond to an event.

Claims closed without payment are often a victory

A major point of confusion is industry data showing that many reported claims close with $0 paid in indemnity. The National Association of Insurance Commissioners (NAIC) reported that just 26% of cyber insurance claims in 2025 were closed with indemnity payments. By comparison, Verizon reported that just 55% of cyber insurance claims in 2025 had a recorded pay-out.

These statistics are easy to misunderstand. Critics label claims that close without an indemnity payment as “denials,” but in reality they reflect the value of cyber insurance in providing support during a crisis. That support can help investigate and contain an incident, potentially reducing disruption and costs.

Cyber incidents are inherently uncertain. Unlike a fire, where a business can visually verify destruction, a cyber insurance claim often starts with the detection of suspicious activity. Calling the claims hotline early can serve two purposes:

- Satisfy reporting requirements in the insurance policy

- Unlock pre-claim legal and forensics practitioners who step in to investigate

If the threat is mitigated before significant damage occurs, costs may remain below the policy retention and, thus, not trigger a payout. A forensics team may also quickly determine the suspicious activity is benign.

Critics label claims that close without an indemnity payment as “denials,” but in reality they reflect the value of cyber insurance in providing support during a crisis.

Rather than billing a policyholder for a few hours of legal or forensics work, Coalition’s Active Cyber Policy includes Rapid Response Services, providing immediate access to critical resources that are not subject to any retention or limit. This helps explain why 64% of Coalition’s closed claims in 2025 resulted in no out‑of‑pocket loss for policyholders in 2025.

Retentions and limits reflect intentional risk choices

Legitimate data on the extent to which an incident is covered by insurance can also get twisted. Critics often point to a NetDiligence report that noted "insurance covered only 32% of total incident costs" across thousands of historical cyber claims.

Out of context, this may sound alarming, but the details matter: 11% of claims in the report remained open, which means the payouts could still rise. Furthermore, the real explanation for this data is rooted in the art of buying insurance.

Enterprises retain more risk

Larger organizations maintain healthy balance sheets and often deliberately select higher retentions (averaging ~$1.8 million) to absorb the cost of most incidents. By design, insurance pays an average of 27% of their total incident costs.

SMBs transfer more risk

Small and midsize businesses (SMBs) typically select lower retentions (averaging ~$41,000) and rely far more heavily on insurance. As a result, insurance covers 69% of SMB incident costs.

When critics claim cyber insurance "only covers a fraction of the cost," they may be confusing a buyer's choice of retention and limit with an insurer’s failure to pay.

Under insurance is a persistent issue

Some cyber losses are so extreme they blow through the policy limit. For example, one claim of more than $500 million caused the average incident cost among large businesses paid by insurance to fall from 42% in 2020 to 13% in 2021. In 2022, two claims of more than $100 million each drove the average SMB incident cost paid by cyber insurance down to 44% (vs. 86% and 83% in 2020 and 2021, respectively).

When critics claim cyber insurance "only covers a fraction of the cost," they may be confusing a buyer's choice of retention and limit with an insurer’s failure to pay.

This is why businesses should work with a specialist cyber broker who can help assess their exposure and choose coverage, limits, and retentions suited to their operations and risk profile.

Fabricated stats and AI hallucinations fuel disinformation

Beyond misinterpreted reports lies a complex web of unsupported data. Often, it begins with a website posting a fabricated, possibly hallucinated, statistic without a valid source. Then, the stat is recirculated via citations from other websites, LinkedIn posts, and popular AI tools that now attach references to their output.

Fabricated stats and hallucinations can negatively shape perceptions of cyber insurance and make it harder for businesses to evaluate coverage.

There are numerous articles citing fabricated statistics about cyber insurance claims. Rather than adding hyperlinks that increase their position in search rankings, we’ve summarized the most common ones below:

- “82% of denied claims involved no MFA”: This is attributed directly to Coalition across multiple third-party blogs. This statistic is not only factually incorrect, but also we’ve never published data on claims denials.

- "44% of claims are denied": This phantom stat is attributed to Advisen and widely cited across technology blogs and trade outlets. Advisen (and its parent company Zywave) never published reports on cyber claims denials, as confirmed by their long-standing editors.

- “40%+ of claims denied”: This figure is cited in MSP blogs attributing a massive denial rate to Fitch Ratings. Neither Fitch's 2024 nor 2025 cyber reports contain this figure, as confirmed by a senior leader of their cyber practice.

Fabricated stats and hallucinations aren’t just a harmless online game of telephone or sloppy AI scraping. They can negatively shape perceptions of cyber insurance and make it harder for businesses to evaluate coverage. Recognizing who benefits most from these narratives makes it easier to call them out for what they are.

How brokers can control the narrative

You don’t need to memorize every report or debate fake internet stats on a client call. The goal isn’t to just dismiss these concerns, but to help businesses understand how coverage may respond and what factors can affect a claim outcome. Consider these responses if your client raises the "cyber insurance doesn't pay claims" objection:

1. ‘I heard most claims close without a payout’

Explain that a $0 payout can be a crisis prevented.

Your response: "Many cyber incidents can be mitigated and resolved with support from incident responders and legal teams, which some insurers will pay for outside the retention (depending on the policy). A cyber insurance policy can provide more than just reimbursement. You're enlisting an on-demand team that helps neutralize threats before they result in a larger loss."

2. ‘I read insurance only covers a fraction of total breach costs’

Clarify the difference between unpaid claims and a business’ intentional choices on retentions and limits, while also cautioning about being underinsured.

Your response: "Coverage depends on the policy’s terms, including retention and limits, and a lower payment doesn’t necessarily indicate a refusal to pay. Many businesses intentionally opt for higher retentions to keep their premium costs down, choosing to handle smaller expenses on their own balance sheet. However, cyber incident costs can be volatile, and it’s important to work with an experienced broker to select the right limit for your business."

3. ‘I saw a stat about how many claims get denied’

Encourage clients to check the source, methodology, and context behind the statistic..

Your response: "It’s worth checking the source of that information. There’s a lot of fabricated data on the internet, and it’s not uncommon for security vendors to try to convince businesses to allocate budget toward software tools instead of insurance. When you look at primary industry reports, cyber insurers pay out billions every year. Just remember outcomes depend on policy terms, the nature of the incident, and the applicable coverage."

Inbox In-Fighting: A Window Into BEC Subculture

Inbox In-Fighting: A Window Into BEC Subculture

If you work in cybersecurity incident response, threat detection, or cyber insurance, you know the standard Business Email Compromise (BEC) playbook.

A threat actor compromises an inbox and immediately sets up email routing rules to hide their tracks and intercept targeted communications. The rules these threat actors create display easily detectable, obvious characteristics.

But what happens when two independent attackers bump into each other inside the same corporate inbox? They don't just quietly ignore each other: they talk. And surprisingly, they often use the names of the inbox rules themselves as a rudimentary chat channel, meaning rules can teach us even more about them than how they redirect email.

These hidden conversations offer a fascinating, rarely seen window into the BEC subculture and provide insights into threat actors' geographies, operational tactics, attitudes, and collaborative dynamics.

How threat actors abuse email routing rules

Abusing inbox rules to evade detection (MITRE ATT&CK T1564.008) is a widespread tactic. Threat actors seek persistent access to compromised accounts to gather sensitive data, launch secondary phishing campaigns, or communicate with high-value targets. In the meantime, they do not want the compromised individual to notice any strange incoming emails or security alerts.

To the attacker’s advantage, most email platforms support automated rules by default. Threat actors can easily exploit this functionality to hide, redirect, or forward emails containing flagged keywords, such as “invoice” or “confidential.”

Inside the rule name chat room

When multiple attackers compromise a high-value target, their paths can overlap. One threat actor might delete another’s hiding rules or launch a clumsy attack that risks exposing the compromise for everyone.

Less commonly observed, individual threat actors may actually communicate within the name of an inbox rule, as Coalition Incident Response* (CIR) recently documented in an investigation:

The recorded threat actor is observing, calculating, and highly aware of the other intruder's presence. They act independently but appear willing to work together to ensure neither ruins the payout.

Sometimes, the interactions are far less polite. In another incident, CIR observed a prolonged, heated argument between two threat actors fighting for control over an inbox, all documented in a sequence of rule names.

Note: The following sequence contains the exact rule names created by the attackers, featuring heavy use of language consistent with Nigerian Pidgin and similar regional vernaculars:

Takeaways from attacker activity

Beyond the sheer novelty of attackers fighting across inbox rule names, these chats provide important threat intelligence:

Geography and Culture

The heavy use of Nigerian Pidgin ("Make We Nor Cast" / "Owfa Wetin De") strongly ties this specific activity to well-documented West African BEC rings, although language alone does not establish the threat actors’ location or identity. "Nor cast the box" can be read as "don't expose or ruin the compromised inbox," while "chop this box" means to profit from it.

Tactics and Target Casing

The attackers aren't just blindly sending invoices; they often study the organization's specific approval workflows. One threat actor explicitly notes that wire transfers require CEO approval via an online portal, but ACH transfers do not require administrator approval. They are doing their homework once in the inbox. The time this takes can benefit the defender, and it also shows the importance of non-IT protections such as secure payment approval workflows.

Tradeoff Analysis

"Make U No Use $12K Job Spoil Dis Box Naw." The attackers are actively weighing the risk vs. reward. One actor is warning that the other is attempting a "small" $12,000 scam that might alert the victim and ruin a potentially much larger payday.

Email providers should adopt a “secure by default” posture

The fact that threat actors can comfortably create dozens of rules with full-sentence names, explicitly routing "invoice" and "wire" emails to the RSS Feeds folder, is a serious gap in default platform security.

Microsoft and other major email providers have the telemetry to see this happening at scale, yet organizations remain vulnerable to the exact same rudimentary techniques year after year. In most organizations, legitimate rule creation is rare, and legitimate use of some of these built-in folders is even rarer.

If we want to actually materially reduce BEC, providers need to adopt a "secure by default" posture. For example:

- Disable inbox rules by default. Most users do not need complex, custom email routing rules. Email providers should make this an opt-in feature that requires admin approval, and keep an eye on the much-reduced attack surface.

- Alert on or even block sentence-length or nonsensical rule names. No legitimate user names a rule BRO IF U RELAX AND AGREE TO WORK WITH ME WE GO CHOP THIS BOX. Or a few commas or periods (,, or ..).

- Alert on payment-related criteria. A rule automatically moving emails containing "ACH", "Wire", or "Invoice" out of the primary inbox should immediately trigger a high-severity alert, in the spirit of risky Sign-ins and other Entra protections.

- Disable common "hiding places" by default. Threat actors love routing emails to the RSS Feeds or Conversation History folders because users rarely check them. These should be disabled by default, and even when available, should only accept routing from known-approved sources (not via rules).

- Disable external forwarding by default. Whether via SMTP or an inbox rule, auto-forwarding corporate mail to an external address should be blocked out of the box.

Threat actors are quite literally chatting with each other in our inboxes because they feel safe doing so. If teams or users really need these features, their administrators can turn them back on, in the meantime improving baseline defaults. Until the platforms we rely on start aggressively blocking these easily detectable evasion techniques by default, BEC will continue to drive cyber claims.

Start monitoring for suspicious activity

We believe that email providers play an important role in systematically reducing the popularity of BEC among cyber attackers. But we also know that a sweeping change to inbox rule features is likely not coming tomorrow. In the meantime, businesses should take appropriate measures to reduce their risk and mitigate suspicious activity before it escalates to financial loss.

To prevent the fraudulent transfer of funds, every second counts. Wirespeed Automated Detection & Response (ADR) is designed to contain threats in milliseconds and automatically take action when suspicious mailbox rules are created. Wirespeed ADR uses deterministic logic to confirm threats as malicious and reset credentials via API to block further activity — with 99% of detections resolved in under 754 milliseconds.**
